A drama that went viral for its daring ‘brain change’ premise wrapped with a jaw-dropping, all-time shock ending.

The May 3 finale of TV CHOSUN’s weekend miniseries Doctor Shin hit a nationwide rating of 2.3% with a peak minute of 2.5% (Nielsen Korea), marking a series high. Launching with the concept of a genius doctor challenging the realm of God, Doctor Shin kept viewers glued to their screens to the very end with twists that went beyond imagination.

In the episode, the body of Geum Bara (Joo Se-bin), who had received Kim Jin-ju’s (Cheon Young-min) brain, ultimately died, leading to her funeral and cremation. Momo (Baek Se-ra), who carries Geum Bara’s brain, watched the grief-stricken Ha Yong-jung (Ahn Woo-yeon) and felt heartbreaking sorrow. In a particularly chilling scene, as prayers were being offered before Geum Bara’s urn, Kim Jin-ju’s spirit appeared and mockingly stuck out her tongue at a monk who told her to “let go of your obsession and move on.”

Tension spiked as the shocking truth surfaced that Shin Ju-shin had orchestrated the chain of events that led to Kim Jin-ju’s brain failing. Momo then confessed to Shin Ju-shin, “I had a baby. It’s a boy.” After seeing the photo, Shin Ju-shin realized the child was Ha Yong-jung’s bloodline, adding more intrigue.

At the same time, Kim Jin-ju’s biological father, Kim Gwang-cheol (Cha Kwang-soo), learned that Momo was no longer his daughter and realized she had undergone the brain change to escape him, sending him into shock. Enraged, he confronted Shin Ju-shin about the brain-transplant scheme, and in the end, he stabbed him with a weapon, bringing Shin Ju-shin to a tragic end.

Paul Kim (Ji Young-san), who returned to Korea, discovered through a letter Geum Bara left and a DNA test that she was his long-lost daughter, and he broke down in tears as he held his grandson. After hearing this, Ha Yong-jung also held his son and told Momo, “Better with Dad than with an aunt, right?” signaling his resolve to raise the child himself.

Later, Ha Yong-jung and Momo revisited the naengmyeon (buckwheat-cold-noodle) shop they had gone to with the then-pregnant Geum Bara. “I really enjoyed the meal that day,” Momo admitted, quietly revealing the hidden truth and leaving a lump in viewers’ throats. But in the climactic ending, when Momo told the golden retriever in the yard, “I love you,” the dog suddenly transformed into the late Shin Ju-shin — an unprecedented moment that flipped living rooms upside down.

Right after the broadcast, viewers exploded with reactions like, “Beyond imagination,” and, “You can’t make sense of this story.”
